If you wish to withdraw your student from OK Homeschool Bands and Strings you will need to fill out this withdrawal form for each student you are withdrawing. Multiple forms may be required. By completing this form, OHBS administration will be notified and forward your withdrawal notification to your location’s band director. No refunds will be issued, as tuition is billed by the semester. Billing will be stopped after receiving your withdrawal form. If you are an Epic Charter School student, we require 45 days notice of withdrawal. Epic is billed one month in advance due to the length of time it takes for Epic to process invoices. If our invoice has already been submitted, no refunds will be issued to you or your learning fund. However, we will stop all future learning fund invoices.
